Challenging times need caring people! We are looking for caring and experienced people to support our residents following hospital discharge into the Home to Decide team.
Our recently formed hospital discharge team is looking for 16 Care Rapid Responders to respond to the current care needs in our community.
What does an average day look like?
- You will be able to show your passion and use your previous care experience to support our residents in their own homes after a hospital discharge, to be safe and to live as independently as possible by assisting them with their care needs.
- If you like variety and a fast-paced role, this could be the job for you. You will be required to react to lifeline alarms, deliver personal care, and assist individuals who have been discharged from hospital.
- You will record progress care plans, log reviews and incident forms, making sure that they meet Council guidelines, referring issues or concerns as necessary in accordance with Safeguarding Procedures in order to protect vulnerable people.
- You will be allocated residents within South Gloucestershire and it will be your role to visit them in their own residential homes. We offer a response service 7 days a week between the hours of 7.00am – 10.00pm and your guaranteed contracted hours will be on a rota basis.
We have 2 rota shift patterns available:
• 4 days on 4 days off. Split shift 07.00-12.00 and 18.00-22.00.
• 4 days on 4 days off. Day 1+2 = 07.00 – 17.00 and day 3+4 = 12:00-22:00
